When a Deep Infection Persists

Even if a tooth has already received one or more root canal treatments from your regular dentist, a stubborn infection can persist right at the bottom of its tip (in the bone). If nothing is done surgically, the actual tooth almost always has to be removed.

  • Hidden Bacteria: Sometimes bacteria reside in tiny ramifications at the root tip that cannot be cleaned via the normal access from above (through the tooth).
  • Bone Damage: If the deep-seated infection is left untreated, it can dissolve the surrounding jawbone and even endanger the healthy neighbouring teeth.
  • Threatened Tooth Loss: A timely small surgical procedure can often protect you from losing your own tooth and needing expensive replacement.

Our Approach

Gentle and Precise

In root tip resection (apicoectomy), we remove only the small, diseased tip of the root to clean the infection cavity in the jaw. Because every millimetre counts here, we work with strong optical magnification (surgical loupes).

With this strong magnification, we can act particularly carefully. We clean the fine root canals at the very bottom (from the bone side) and seal them as bacterially tight as possible to prevent renewed infection.

We remove the infected root tip, thoroughly disinfect everything, and seal the tooth canal from below with a high-quality material.

You receive precise tips for rest at home. We see you again a few days later for wound inspection at the practice and check the healing progress.

Important Questions About Apicoectomy

The effort is often noticeably worthwhile for you. Keeping your own tooth in the dental arch frequently spares you elaborate treatments with bridges or implants. Visually and functionally, your own tooth remains the most natural solution.
The surgical part itself is generally low in pain thanks to modern anaesthesia. However, in the first days of healing, mild wound pain will present itself, similar to after having a tooth extracted. Through cooling and standard painkillers, this healing phase can usually be very well controlled.
If root canal treatment has failed, this operation is the proven standard approach to preserve the tooth short of extraction. If a resection is no longer feasible, extraction is often the only remaining option.
The root tips and nerve canals in the tooth are incredibly tiny, sometimes as fine as a hair. If work is done there with the naked eye only, the probability of not completely eliminating bacteria increases significantly. Optical magnification through our surgical loupes is therefore very important for healing success.
A standard, medically approved root tip resection is initially covered by health insurance. If modern supportive measures are additionally desired (such as supportive 3D planning), we inform you in detail about your out-of-pocket contribution.
