What Are White Spots?

White Spots — Harmless but Visible

White, opaque areas on teeth are a widespread aesthetic concern. They occur when the mineral structure of the tooth enamel is altered in places. The causes are varied.

  • After Braces: Fixed brackets make cleaning difficult. White, demineralised areas frequently form around the bonding sites and become visible after the braces are removed.
  • Fluorosis: Excessive fluoride intake during childhood can lead to whitish enamel changes that spread across the entire tooth surface.
  • Developmental Enamel Disorders (MIH): Molar-incisor hypomineralisation is a structural disorder of the enamel that manifests in whitish-brownish spots — particularly on molars and incisors.

All these changes affect the light refraction of the tooth enamel: the porous areas reflect light differently from healthy enamel and therefore appear brighter.

The Principle

Filling Pores, Matching Appearance

Enamel infiltration uses a specially developed, low-viscosity resin that penetrates through capillary forces into the porous structures of the altered enamel and fills the cavities.

Since the infiltrant refracts light similarly to healthy tooth enamel, the white spots visually disappear or become significantly less noticeable. Natural tooth substance is not removed — the procedure maximally preserves healthy tooth substance.

Cleaning and Preparation

The tooth surface is thoroughly cleaned. If necessary, the treatment area is isolated with a rubber dam (cofferdam).

Pre-Treatment (Etching)

A special gel opens the outer enamel surface so that the infiltrant gains access to the porous areas.

Infiltration

The low-viscosity resin is applied and penetrates deeply into the pores of the altered enamel through capillary forces.

Curing and Polishing

The resin is cured with a special light lamp. Finally, the surface is polished to a high gloss.
